Procedure
What is it?
The
Public Subscription License is issued under the provision
of Public Collection Ordinance 1996. It permits the holders
to collect donations or raise funds from the public. Application
for a license covering the Kapit Division will be considered
by the Resident. If the public subscription is to be done
in two or more divisions, the application must be submitted
to the State Secretary, 20th
Floor, Wisma Bapa Malaysia, Petra Jaya, 93502 Kuching.
How to Apply?
- Obtain Form A free of charge
at the Resident’s
Office
- Fill form A in four (4) copies and submit them with following documents:

Three
(3) passport sized photographs of each applicant
and collector |

One
(1) photostat copy of identity card of each applicant
and collector |
| - |
One
(1) copy of bank statement of account |
| - |
One
(1) copy of the last minutes of meeting |
- Fee of RM25.00 will be charged for the license. |
